On your first visit, you will be asked
to provide your insurance card(s) and a photo ID. You will also
be asked to complete a new patient
registration packet which you can print from this
website. If you choose not to print these forms and complete
them in advance, you will be asked to arrive in advance of your
appointment time to allow for the completion of this paperwork.
We will verify this information at all follow-up appointments
.
If there is a change in your health insurance or demographic
information, we ask that you let us know as soon as possible.
Please familiarize yourself with our medication refill and financial
policies which are also available on this website as well as
part of your
new patient registration paperwork.
At every appointment, please be prepared to provide us with
an updated list of your current medications as this is an important
part of your treatment at our office.
As stated in our financial policy, all co-payments are due at
the time of service.
