If you have a question as to whether or not we participate with your insurance plan, please contact your insurance directly. Some of the primary plans with which we participate are:

Many insurance plans require that the
patient meet a deductible, (especially for diagnostic testing
like EMG’s or sleep studies) before they will pay anything towards
a claim. We ask that you familiarize yourself with your plan’s
coverage and benefits, and if necessary call our office to make
payment arrangements. All co-payments for new patient and follow-up
office visits are required at the time of service. |
